StructureDefinition.id shall match final part of canonical

Description

A number of profiles have a StructureDefinition.id that does not match the final part of the canonical URL for that profile. This trips up the IG Publisher in some cases. There seems to be no good reason for keeping those StructureDefinition.ids as-is, so we better make it a rule they match.
