Mapping nodig van FHIR allergy-intolerance-category naar AllergieCategorieCodelijst

Description

De zib AllergieIntolerantie schrijft een categorisering via AllergieCategorieCodelijst voor. In FHIR komt dit conceptueel overeen met AllergyIntolerance.category, en in het profiel is het concept daar ook op gemapt.
FHIR schrijft echter een eigen verplichte codelijst voor, allergy-intolerance-category. Deze komt niet een-op-een overeen met AllergieCategorieCodelijst. Om FHIR te implementeren, hebben we in beide richtingen een mapping nodig tussen de twee.

Verduidelijking van Impact

No expected impact other than relieving implemented from the burden of having to find out the mapping for themselves. The ConceptMap may be seen as enhanced documentation

Proposed solution (NL)

Via ZIB-998: add ConceptMap (+ marker in profile zib-AllergyIntolerance for its existence) that outlines:

418471000 equal food
419511003 equal medication
426232007 equal environment
419199007 subsumes biologic
OTH unmappable

For OTH add comment: The HCIM defines that 'other' is a possible value for the AllergyCategory. The required AllergyIntolerance.category binding does not recognize this use case. If transmitting the value under OTH is not required, omit the AllergyIntolerance.category. If the actual value associated with OTH is known and relevant, use AllergyIntolerance.category + extension http://hl7.org/fhir/data-absent-reason 'unsupported' + extension http://nictiz.nl/fhir/StructureDefinition/code-specification with the description under valueCodeableConcept.text

De binding in FHIR is required, dus "To be conformant, codes in this element SHALL be from the specified value set."

De valueSet in de zib kent echter ook 'OTH', hoe ondersteunen we dat dan in FHIR?

Former user 'Voeding', 'Medicijn' en 'Omgeving' van de ZIB komen m.i. een-op-een overeen met 'food', 'medication' en 'environment' van FHIR. Maar de ZIB-waarde 'Allergy to substance/Stof of product' komt niet goed overeen met een van de FHIR-waarden, en andersom komt de FHIR-waarde 'biologic' niet goed overeen met een van de ZIB-waarden.

In welke situatie is deze niet te mappen? Ik heb de codelijsten vergeleken en ze komen aardig overeen.

Volgens mij is het belangrijkste (Propensity to adverse reactions to drug) wel te mappen naar geneesmiddel allergie.

Ik vraag mij af of de overige al in praktijk te downloaden zijn van ziekenhuizen.
