Add root mapping to LaboratoryTestResult-Observation profile

Description

The root comment in the zib-laboratorytestresult-observation profile states the following:

This profile is used for two concepts of the HCIM LaboratoryTestResult at te same time: the root concept (NL-CM:13.1.1) and the LaboratoryTest concept (NL-CM:13.1.3).

1) The former concept however, is not present as a mapping on the root, and this should be added;

2) The typo 'te' in the comment above should be fixed as well.
