Change comments in Questionnaires FHIR profiles

Description

The comments in the MedMij Questionnaires FHIR profiles contain sentences like 'Nictiz has chosen' and 'the Nictiz situation'. Those comments should be changed so they are aimed to the Dutch situation/realm.

To aim for a more general approach in the Questionnaires information standard, textual changes were made in the Questionnaires profiles, README and the Simplifier project so that they no longer state 'MedMij' and 'Nictiz situation'.
