Update ConceptMap equivalences in R4 profiles

Description

The equivalence element in the ConceptMaps is not always correctly filled (mostly due to the not straightforward phrasing within FHIR of the possible values). These incorrect values need to be changed.

Pas ConceptMaps aan zodat het equivalence-element correct is gevuld.

Update the ConceptMaps such that the equivalence element is filled correctly.

De equivalence-elementen in de ConceptMap van AdresSoortCodelijst naar AddressUse zijn gecorrigeerd. Verder zijn er tekstuele verbeteringen doorgevoerd in alle ConceptMaps.
