      Added IHE.FormatCode.vs ValueSet to the package, which is referenced in the IHE MHD STU3 profile (which was already included).

      No value set found at DocumentReference.content.attachment.contentType (url = 'http://hl7.org/fhir/ValueSet/mimetypes')
      No value set found at DocumentReference.content.format (url = 'http://ihe.net/fhir/ValueSet/IHE.formatcode.vs')

      These are from the missing IHE MHD base spec for STU3. In addition to the profiles we recovered we also need to recover these ValueSets (and potentially CodeSystems) to complete the set
