      Description

      The nl-core-patient profile uses the HL7 core extension patient-nationality. However, the ValueSet binding to NationaliteitCodelijst is done on the root of the extension, not on the code slice.

      Commit is done in branch MM-1002. However, this change also fixes the system from the ValueSet, which defeats the purpose of making a binding extensible. Furthermore, it restricts the cardinality of code to 0..1, which may be backwards incompatible.
